The below information-request form is for publicly advertised events/functions that will occur in CANADA or MEXICO (or Bermuda, The Bahamas or Jamaica).
WHO SHOULD USE THE BELOW INFORMATION-REQUEST FORM -- Complete and submit the below information-request form if all of the following are true -- 1. Your event will take place in CANADA or MEXICO (or Bermuda, The Bahamas or Jamaica), 2. The performance of the headliner (the main artist) will be promoted (and open) to the general public, 3. You have $50,000 USD or more to pay an artist for each date, with a separate budget for artist's expenses, 4. You can provide a valid e-mail address that you check regularly, and 5. This is NOT a request regarding a theatrical production, film, talk-show, sit-com, commercial, product endorsement, record deal, interview, or non-compensated "invitation." (If one or more of the above is not true, start over. Click HERE.)
E-MAIL PRIVACY POLICY -- We will only use your e-mail address to correspond with you regarding your specified needs. We do not put your address on a marketing mail list. We never sell, distribute or use e-mail addresses for mass marketing. We are very anti-spam. You must provide a correct/valid e-mail address to receive our reply. Please type your address carefully.
ALL CAPS -- Continuously typing in ALL CAPITAL LETTERS is considered SHOUTING at the reader - and is much more difficult to read. Be kind to us. Please do not SHOUT. Release the "Caps Lock" key on your keyboard. Thank you for this consideration.
NAVIGATION -- Most users will be able to use their keyboard "Tab" key to navigate this form.

Minimum budget requirement for this form: $50,000 USD or more, with an additional budget for artist's expenses/costs.
Will the artist's performance be advertised/promoted (television, radio, newspapers, Internet, etc.) and open to the general public? Yes No
The minimum, acceptable entry requirement is "$50,000 USD or more." An entry of "$50,000 or less" is not sufficient.
Non-numerical entries cannot be considered. Do not enter any of the following: "tbd" or "to be discussed" or "will discuss" "n/a" "open" or "negotiable" "it depends" "unknown" or "don't know" or "???" "call me" or other similar non-numerical entries.
Prior to submitting this form, please determine how much you can allot for the artist's talent fee.
Enter the actual US $ amount that you are prepared to pay for the headliner or for this artist - or the range of your budget. If this figure is too low for the artist you seek, we will not be able to proceed. Note: If your budget is $150,000 - $250,000, it is best to enter this amount. If you enter only $50,000, we will not discuss artists whose fee exceed this. Is the above an actual numerical figure or range? Example: $100,000, or $400,000-$500,000 Yes No
Is the above entry displaying (or suggesting) any dollar figure less than $50,000 USD? Example: $25,000-$50,000, or $50,000 or less Yes No
(For the budget entry for multiple artists, see below.)

I have read and agree to the binder requirement terms: Yes No Do not understand
If this is a public concert, is this your first promotion? Do you have any prior experience? For example: "first time for any concert" "first time for artists at this level" "moderate experience" "very experienced and knowledgeable" "My 1st time, but my boss/company is very experienced."
What significant artists have you previously promoted? When? Where? (A brief summary is acceptable.)
For example, who is paying for the artist(s)? You (or your company)? Outside investors/sponsors? Ticket sales?
Do you have immediate or direct control of 50% or more of the required funds for payment? Or, do you have to convince investors to give you money? Immediate control means the funds are in the buyer's account, ready for bank-wire.
If you entered a budget figure of less than $50,000 USD for the talent fee, or you are primarily seeking acts under $50,000, stop. Please do not continue. We are unable to provide you with the services you seek -- and we sincerely wish you success as you seek services elsewhere.
The below statement asks you to confirm your budget figure entry. Clicking YES means confirmation.
Do you confirm that your above talent-fee budget entry to pay the artist is $50,000 USD or more – and that you have a separate/additional budget to cover expenses?
