The form you are using is only for events that will take place outside of the U.S. or Puerto Rico. If your event is to occur in the U.S., you are not using the correct form.

For events in the United States --

  a.) If the artist's performance is going to be advertised and open to the general public, such as for a public concert or festival, click HERE.
