Our browser-based FIRM OFFER FORMS (following this page) are designed to be printed and then filled in by hand. These forms are formatted to have no more than three printed pages.

However, different browsers might have different page settings that will have an adverse effect on the print-outs.

Your particular browser might have slightly different instructions to do a page set-up. Use whatever settings might be available to you to minimize margin settings (and eliminate headers and footers) prior to printing.

The following should work for most users.

BEFORE printing a form, perform a PAGE SETUP now to minimize printed margins.

For example ---


If you are using an Internet Explorer browser ---
1. From your browser window menu, select...
     FILE > PAGE SETUP...
2. Delete all settings for header and footer.
3. Set margins:
     Left and Right: 0.5    Top and Bottom: 0.5
4. Click OK.

(Your computer might automatically change these settings for your particular printer.)

If you are using a Netscape browser ---
1. From your browser window menu, select...
     FILE > PAGE SETUP > MARGINS & HEADER/FOOTER
2. Set all header and footer settings to: --BLANK--
3. Set margins:
     Left and Right: 0.5     Top and Bottom: 0.5

(Your computer might automatically change these settings for your particular printer.)


If you do not have access to the above options, the printed format might appear odd, or more than four pages will be printed. However, you can still use the forms.


Definitions:

Public -- if open to the general public and the artist's performance is to be advertised in the public media (such as television, radio, newspapers).
Private -- if not open to or advertised to the general public. For example, attendance is by invitation only or is restricted to a specific group, and the event is to be promoted privately such as by word of mouth, trade paper, company or school paper, bulletins, letters, flyers, etc.


If you are hiring an artist to perform multiple times, and at least one of the performances is being advertised to the general public, use the PUBLIC form.
